Hospice of Northwest Ohio served its first patients in 1981 and is the Toledo area's largest provider of hospice care. A non-profit, community-based organization, we are the only area agency solely dedicated to caring for the terminally ill. In 2008, we served over 2,800 patients and families in their own homes, in nursing homes and at
our freestanding Hospice Centers. We are Joint Commission accredited and Medicare/Medicaid certified. No one is ever turned away due to inability to pay.

Our mission: Hospice of Northwest Ohio provides specialized medical, emotional and spiritual care to people of all ages--and their families--living with any end-stage illness in Northwest Ohio and Southeast Michigan.

Patients are usually referred to Hospice by their physicians. However,referrals also can be made by family members, friends, clergy, hospitalsocial workers, discharge planners or others. Patients and families areencouraged to call Hospice of Northwest Ohio directly ? whether or notthey have a physician referral ? to seek advice and information at anytime during their illness.If it appears hospice care would be beneficial, the Hospice staff willwork closely with all the appropriate people to help facilitate areferral and admission to our program. Meetings to discuss eachpatient's unique needs can be arranged at a time and place that isconvenient for family.
